Testimonial form examples

Testimonial collection forms are used to easily collect text and video testimonials from your happy customers. They make it easy for your customer to share what they love about your business, and are the first step in generating more word of mouth.

Senja customers don't just create one form. They setup different testimonial forms for different types of testimonial.

Our power users even use forms to collect different marketing and sales materials: like case studies and content marketing.

In this post I am going to share 10 examples of testimonial forms, but first let me share with you what pages every testimonial form should have:

Testimonial form pages

The best testimonial forms have four key pages:

The form landing page

This is the page that introduces the request, and explains how the testimonial will be used. This is a great screen for a personal Welcome video from a real person on your team. You can also use this screen to highlight any rewards you're offering in return for a testimonial.

The testimonial collection page

This page is where you collect the testimonial itself. This can be a web-based video recorder, an upload page, or a simple form field for collecting text testimonials. This page also contains your testimonial questions.

The customer info page

This page will help you collect information from the customer: about them and their business. This information has two key uses. First, you share it with the testimonial so that it's more authentic. Second, you use it to help you and your team find the most relevant testimonial for each lead.

The thank you + word of mouth page

This is the most powerful but most neglected testimonial form page. You should have a page that thanks your customer for leaving you a testimonial, and encourages them to share it online.

This is called Word of Mouth marketing and it's an extremely effective marketing channel. You can get your existing customers to share what they love about you, and drive new leads to your website and product pages.

Do this continuously, and your happy customers can do your marketing for you.

Bonus: the "You've got a testimonial" email

When a new testimonial arrives, make sure you and your whole team know about it. Trigger an email and a Slack alert so that it's available and can be shared in the right places to grow your business.
